That is a Skype service. Try to update program or better yet uninstall and reinstall again.
Oh right! Only started happening after the Avast update. Must admit though when I start Skype the dialogue below comes up, I think since the merging of Windows Live Messenger and Skype (not sure I rarely used Skype before). It works ok once OK is pressed though v. slow.

Also the dialogue on closing down seems to have gone (I pressed Cancel to debug after which it closed cleanly).

I had intended to reinstall Skype due to the error below so I'll do that and see what happens. Thanks

I uninstalled and reinstalled Skype. I no longer get the message as http://forum.avast.com/index.php?topic=112991.msg885707#msg885707 (even before doing this) but I am still getting the same message as per the previous post on launching Skype. Am sometimes getting a similar message as I close Outlook 2003 (about writing to memory). Will try to capture it.

Ntdll.dll errors can be anything in your sys. You said that not only you get them with skype but also with Outlook so it is better to take a look at your programs, sys, and hardware. This last one can happen rarely, but it might.

This is a guide to diagnose ntdll.dll errors. Also if you google it there are many more.
