Almost all modems ISP's are providing are also routers and firewalls nowadays.

I'd think if it's clear I have no router, it should also be clear that I must be connected to the net via this long, skinny thing called a "wire".

No it is not...

i have a router ... some computers are connected with wire and some are wireless 
i can also connect direct to the cable modem ... with wire.
And there is something called 3G/4G modem, they can also be built in to the computer
and then there is the old dialup modem

Often routers connect your LAN with a DSL-cable to the Internet. Your box instead connects to TV-cable and is called cable modem.
Besides the different wires they work similar looking from a distance.
So the message "router is open to hackers" may be right, as smart scan checks the security of that box.

yeah. when I click "Router settings" it goes to an unavailable web page. I think it's just a false positive.

If it's an address like http:/10.1.10.1 it's actually trying to open your router interface so you can make the needed changes to secure it.
