Win32:Trojan-gen. {UPX!} in C:\windows\tvtmd.exe

I have tried to eradicate this file in Windows and in Safe Mode as well.  In Safe Mode, I'm able to delete it, but in Windows it reappears, I go back into Safe Mode again and the file isn't there in the Windows folder, but when u go back to regular Windows the file is still there, and I can't do anything with since it says that Windows is using the file.  I've exhausted everything I know ( which isn't alot mind you).  So I turn to you guys, help me please.

First of all, it is a kind of Spyware You have some choices to get rid of it. Download adawrae(www.lavasoftusa.com) or/and SpybotSD(security.kolla.de) install, update and let them clean it, or post a Hijackthis log here( http://mjc1.com/mirror/hjt/)
