I've twice had obvious fake invoices from Renault as Word docs in email and Avast hasn't detected them. If I test the latest on VirusTotal, I get the result in the file. What is going on?

You just discovered something evryone (most) know. NO security program have 100% detection or zero false positives

Most malware start life as undetected, then some security vendor will be the first to detect it and the rest arrive later
This is not the actual malware but a downloader and it usually download ransomware or emotet banking trojan. In your case it seems to be Emotet
