Avast and Outlook 2003 hang on login + RPC error

- Systemspecs:
Dual Celeron 400
640MB RAM
Windows 2000 Professional SP4 UK
All hotfixes installed, except:
KB835732 (causes lots of PlugPlayManager events in Eventviewer, and causes hanging on loginscreen on many PC's)
and KB833407 (Bookshelf Symbol 7 font update)

- Software installed
Avast! 4.1 Free Home Edition EN
Office 2003 Professional NL (fully up-to-date)
Kerio Personal Firewall 4 EN
Mozilla Firefox 0.8 EN
Internet Explorer 6 EN
RAMDisk 2.2
No other relevant software.

- Problem
System hangs a while right after login and the Outlook/Exchange resident provider refuses to start.

- Symptom 1
After booting and login, Outlook 2003 refuses to start, and Avast! is also not starting (the blue icon in the systray is crossed out) It takes both applications about 80 or more seconds to start after login.
In the meantime, aswUpdsv.exe, ashServ.exe, ashmaisv.exe, ashDisp.exe and OUTLOOK.exe all show in the taskmanager. When I hover above the crossed out Avast! icon in the Systray, it says: "avast! On Access Scanner: 0 provider(s) total, 0 running". There is no extraordinary CPU activity by any system or application process. After some 80 or more seconds after login, it seems that first Outlook starts, and then Avast! 2 seconds later, excluding the Outlook/Exchange resident provider. All other programs run perfectly normally directly after login, except Outlook 2003 and Avast!

The Eventviewer shows the following problem in the System log:

The avast! Antivirus service hung on starting.

- Symptom 3
Reading back in Eventviewer, I can see that this problem started on the moment that I installed and updated Office 2003. Office was installed after Avast!

- Symptom 4
In the On-Access Protection Control (with details ON), the Outlook/Exchange resident provider is greyed out. Under the title, it says:  The provider is waiting for a subsystem to start.

- What have I tried
Tried reinstalling Avast! After installation and reboot, I get the "The AAVM subsystem detected an RPC error" error once. After that I dont get any visual popups anymore.
Tried starting up without: Kerio Firewall service, messenger service, telnet service, QoS, Routing and Remote access service, Automatic Update service, e.a.
Tried rebooting with the Outlook/Exchange resident provider disabled.
Tried booting with all the resident providers disabled (except Standard and Outlook/Exchange)
Tried watching the registry in realtime for obvious errors
Watched / watching the following somewhat similar threads on this forum:

Outlook / Avast hangs
I tried setting the Avast! logviewer settings to "debug". I only get the following messages, under Info, the others are all completely empty:
