If avast isn't detecting it as infected ?

Then there is little point in adding it to the exclusions lists as that will have no impact at all. avast! scans and alerts to infection, but it doesn't block, so there is something else going on.

What is the full text of the access violation error ?
Unless it were accessing avast's files or registry entries then avast wouldn't be involved in an access violation (IMHO).

Have (or did) you another AV installed in this system, if so what was it and how did you get rid of it ?
