This hasn't changed either - Any malware that manages to get past your defences will have free reign to connect to the internet to either download more of the same, pass your personal data (sensitive or otherwise, user names, passwords, keylogger retrieved data, etc.) or open a backdoor to your computer, so outbound protection is essential.
As you are aware not every infection comes from the internet and no single program so it doesn't have to negotiate your router/firewalls inbound protection and any outbound connection from your system allows the co-responding inbound connection in unmolested, so you are hoping your AV detects whatever that might be; so outbound protection is essential IMHO, especially for your average user.
