I got this and avast did not even stop it. Even the scans I run told me I did not have any infections. Why didn't avast catch this?

Thats because no antivirus solution can detect everything, it's just impossible. The malware writers are always one step ahead of AV companies because they have to discover them. Avast's detection is around 97% from what I know.

My suggestions:
 - Pair up your antivirus product with Malwarebytes and Hitman Pro. They work great alongside
   antivirus programs and they're free.
 - If you have avast Pro or Internet Security, use the Process Virtualization module to prevent any future infections.
   Or use Sandboxie if you want a free version (a bit harder to use imo).

Well the thing is, fake antiviruses are known to keep changing their code to avoid antiviruses from detecting them again, so they are especially tricky to deal with.

Would winpatrol plus with real-time monitoring of startup programs and registry monitoring enabled prevent this sort of infection?

Would logging on XP with a limited rights user account prevent it?

(though logging on XP other than as an administrator seems to be a real chore - a lot of things won't work, and I can't find any way to gain temporary admin rights through password entry - I have to log out and log in again as an administrator.)

It would detect it in the Free version however the PLUS version will provide much more information.

If you do not install it, i.e., set no to UAC question. No, it won't infect your computer.
But UAC will never say you if the setup is from an infected program or a clean one.
