If they fail it is either a pebkac error (which it is in 99.9% of the cases) or a infection.
In the 27+ years I have been working with computers I never encountered a problem that required anything like hacking/cracking a password like scythe944 seems to be promoting.
There are legal ways to solve the problem!
It is ofcourse very stupid from MS not to force people to set a password on a administrator account. It is making it relatively easy for malware to get full control over a system.
Besides that, people should never use a administrator account, or a account with administrator rights unless absolutely needed. If you are logged in/using such a account, it means that when the system gets infected, the malware has the same rights! Always use a restricted account with a password.
