Hi davexnet,

Your pseudo 'Safe Mode' was likely the native VGA Vista video driver running instead of your onboard Intel or a third-party discrete video driver.  VGA is Microsoft's/IBM's 16 color driver dating back to 1987, so it does not look normal; Safe Mode uses this driver as Windows is running minimal drivers in that scenario.

https://en.wikipedia.org/wiki/Video_Graphics_Array

Likely your normal enhanced video driver crashed on boot, and your system ran VGA instead.  As you say, lucky you got video at all.
Thanks for the info.  Question remains is it related to avast or not? I'm on XP at the moment, I'll log back onto
Vista later and see if there is anything in the event log.  Certainly uninstalling avast restored normal operation, and I was
successfully able to re-apply all the updates..
For now I've left avast uninstalled from there.

One step more:  Use avastclear.exe to completely remove any remnants left over.  https://www.avast.com/uninstall-utility

Be sure to move the downloaded file to your administrator desktop before running this tool.  It will ask if you want to run Safe Mode when you double-click.  Click 'Yes' and your Vista system should automatically reboot to Safe Mode.  Be sure to leave the default cleaner settings alone unless you installed avast! in a non-default folder.  In that case, you would point aswclear.exe to that avast! non-default folder.  If you installed to the default folder as you should you do not need to do anything as it is already set to remove the avast! default folder location.  Select what version of avast! you wish to uninstall.  Reboot when done.

If you've ever run or installed another a/v solution, or Vista came with a trial version you uninstalled later, see:  https://singularlabs.com/uninstallers/security-software/

Download any vendor tools as appropriate.  Reboot after running each tool.  Set a restore point before running any tools to recover if necessary, but should not be a requirement to do so.

If any old a/v's were run and then uninstalled later, then these left-over files could be the source of your issues with avast!.

I installed on 6 machines.....5 with Avast V9 (I'm so lazy I need to upgrade these), one Avast V10.  All recommended MS updates went well and about ten or so optional for MS Office, etc.  All went fine on all machines.

Also, side note......I have new MBAM & MBAE & CrytpoPrevent versions installed too...no issues all the way around.

P.S.  Did not install Skype.....it is on my Kid's iPads.....that is plenty.

Yes I ended up with 21 Optional Updates and Installed also with no issues on W8.1 64 Bit.
I don't recall ever getting so many optional Updates all in one batch. ???

I had 20 optional ones but only chose to install 9 of them. The rest were for things I don't use and never will. Even a couple of the 9 were questionable that I even needed them.

Me too... I've thought it would be an "issue" in a computer I was wiping most programs, but now, in my new laptop, the same...

This update came up and I installed it. It was an optional update. Does anyone here know what this update does? Name of the update means Diagnostics Tracking Service, so it tracks diagnostics service and send them to Microsoft? Is that what it means?

I have 28 optional updates but I'm not going to install them.  When I click on any update and get more information from Microsoft I haven't found any that I really need.  If they switch to important next Tuesday I'll install them.

You've tried running sfc /scannow and didn't receive the error ???
I certainly did but didn't realize till I read this thread what caused it.
I've now uninstalled KB3022345 and hidden that update from future updates.
