Custom is a copy of Default, plus any changes made to it through the UI.  Default is almost the same as Medium.  A diff shows it monitors only two less keys.  I haven't tried it on Light either.  I like the noisiness, but I run it in Alert Mode, which automatically Accepts any changes without wating for my approval.  I use MJRW as a monitor, and WinPatrol for asking my permission.
When I finally get tired of seeing a key in MJRW's report, I use the UI to comment it out.  I've done that only once so far.

MJRW has been educational for me too.  I automatically accept any changes it reports.  They've all been made by legitimate software.  The same with WinPatrol.  When I decide I want to disable a startup program or Automatic service that has been added by a software installation, I edit them with Autoruns or Window's Services Administration Tool.  I've added only one exception so far to MJRW.

If I'm ever hit by a malware attack -- knock on wood -- I expect WinPatrol and MJRW to scream bloody murder.

Edit: And avast! too, of course.

Me I'm of the opinion of, why buy a dog for defence but stop it from barking kind of guy, so I have it on Prompt Mode, which at times gets noisy.

You're welcome, Bob.  I tried WinPatrol because it was recommended here.  It seems to be a keeper.  Thank you all for sharing your experience with it.

Me I'm of the opinion of, why buy a dog for defence but stop it from barking kind of guy, so I have it on Prompt Mode, which at times gets noisy.

Mine barks, but it doesn't bite.  :)
